Privacy policy

Information we collect

• Contact and identification details: name, date of birth, email, phone, address and preferred language of the primary member and household members.
• Membership details: chosen plan, payment frequency, acceptance of the agreement and fee schedule, appointments, benefit usage and balances.
• Payment details: processed by Stripe, Inc. We do not store your full card number; only the brand, last four digits and expiration date.
• Messages you send us through the portal, email or phone.
• Technical data: IP address, browser type and pages visited, used for the security and operation of the site.

Cookies

We only use the cookies needed to keep you signed in to the portal and remember your language. We do not use advertising cookies or sell browsing information.

How we use your information

• To enroll you and manage your membership, including the recurring charges you authorized.
• To schedule appointments, send reminders and answer your messages.
• To send notices about your account, price changes or changes to these terms.
• To protect the site, prevent fraud and comply with the law.

Who we share it with

We do not sell or rent your personal information. We only share it with:

• Vendors who work for us (payment processing, email and text delivery, data hosting), under contracts that require them to protect it and use it only to provide their service to us.
• The clinic professionals who care for you.
• Authorities, when the law requires it or to protect someone’s safety.

Security and retention

We use encrypted connections, per-user access control and private document storage. No system is completely secure: if you suspect someone accessed your account, change your password and let us know.

We keep information while your membership is active and afterwards for as long as Florida and federal law require for medical and accounting records.

Your choices

• You can view and correct your contact details in the member portal, under My profile.
• You can choose how to receive appointment reminders from your profile.
• You can ask us for a copy of your information or for its deletion where the law allows, by writing or calling us.

Children

Minors can only be enrolled by a parent or legal guardian as household members. This site is not directed to children under 13 and we do not knowingly collect their data directly.

Changes to this policy

If we change this policy, we will post the new version on this page with its update date and, if the change is significant, we will let you know by email.
